Bounce House & Games Waiver & Consent

Consent to Participate

I, the undersigned parent or legal guardian of the above-named child, hereby give permission for my child to participate in the bouncy house activities at the Al Huda SPARK event held at Finch Creek Fieldhouse. I understand that participation in these activities involves physical movement and carries an inherent risk of injury, including but not limited to falls, collisions, or other accidents.

Assumption of Risk and Release of Liability

I acknowledge and understand that Al Huda Foundation, Al Huda SPARK, Finch Creek Fieldhouse, and all associated organizers, volunteers, and staff are not liable for any injuries, accidents, or damages that may occur as a result of my child’s participation in bouncy house activities.

I voluntarily assume full responsibility for any risk of bodily injury, property damage, or loss arising from participation. I agree to release, indemnify, and hold harmless Al Huda Foundation, Al Huda SPARK, Finch Creek Fieldhouse, and their representatives from any and all claims, liabilities, or expenses related to such participation.

Medical Consent

In the event of an emergency where I cannot be reached, I authorize the event organizers to seek appropriate medical attention for my child. I understand that I am responsible for any medical expenses incurred as a result of such treatment.
